Return to riding after open reduction internal fixation of acute clavicle fractures in professional horse jockeys
Dr David Duckworth is a co-author of a clinical study examining the outcomes of surgical treatment for clavicle (collarbone) fractures in professional horse jockeys, with a particular focus on recovery, complications and return to competitive riding.
Clavicle fractures are among the most common injuries sustained by professional jockeys, often resulting from high-speed falls during racing. Surgical fixation is frequently used to stabilise these fractures, but there has been limited research into how quickly jockeys can safely return to riding following surgery.
About the Study
The study reviewed 72 professional jockeys who underwent open reduction and internal fixation (ORIF) for acute clavicle fractures between January 2008 and December 2024.
All patients underwent surgery using a limited-incision technique to realign and stabilise the fractured clavicle with a plate. Researchers assessed recovery times, shoulder function, fracture healing and post-operative complications.
Key Findings
The study reported encouraging outcomes following surgical fixation:
- Return to riding: Jockeys returned to riding after an average of 7 weeks, with recovery times ranging from 5 to 16 weeks.
- Shoulder movement: Full range of motion was achieved after an average of 7 weeks.
- Fracture healing: Radiographic evidence of fracture union was observed after an average of 10 weeks.
- Functional outcomes: Patients reported excellent shoulder and arm function at 12 months, with an average Disabilities of the Arm, Shoulder and Hand (DASH) score of just 1.
- Complications: Significant complications were uncommon, with severe post-operative numbness reported in 1.4% of cases and plate irritation in 5.6%.
What Do These Findings Mean?
The findings suggest that surgical fixation of acute clavicle fractures can provide excellent functional outcomes and facilitate a timely return to riding for professional jockeys.
The research also provides valuable information for orthopaedic surgeons managing high-demand athletes, particularly where restoring shoulder function and returning to competitive sport are important considerations.
These findings may help guide treatment discussions and post-operative expectations for professional jockeys and other athletes recovering from clavicle fractures.
