Making an Appointment:
All routine appointments are made by phoning the Parramatta rooms on (02) 9806 3333
Office hours are 8am till 6pm Monday to Thursday and 8am till 5pm on Friday.
**If you have a fracture, you may prefer to attend the emergency department of the Sydney Adventist, the Hills Private or Hornsby hospitals where the staff will be able to provide initial care and contact Dr Duckworth directly for you**
You can also e-mail us with any queries or your request for an appointment by clicking on the “Contact Us” tab above. In doing so, you will be contacted first thing on the next working day.
What to bring to the appointment:
- Referral letter
- Any X-rays, scans or MRI’s relating to your condition
- Consultation fee.
What to Expect:
You will be expected to complete some paperwork at your first consultation.
This includes-
Your contact details
- Insurance/compensation details (including insurance company, claim number, case manager’s name, their direct phone number and fax number)
- Information about your current shoulder/elbow condition
- A medical history form
We invite you to print these forms, complete and bring them with you to your consultation with Dr Duckworth.
